The Center brings the national Program of All-Inclusive Care for the Elderly (PACE) services and programming for seniors in and around Elizabethtown.

(ELIZABETHTOWN, KY) — Care Guide Partners PACE a Hosparus Health affiliate, is looking for highly-qualified physicians, advanced practice registered nurses (APRNs), social workers, registered dieticians, recreational therapists, nurses, and other healthcare professionals to join its growing team. Prospective applicants are invited to a weekend hiring event on Friday, January 19, from 4 to 6 p.m., and Saturday, January 20, from 10 a.m. to 2 p.m., at the new Care Guide Partners PACE Center, 109 Buffalo Creek Drive in Elizabethtown.

Hosparus Health, a nonprofit organization renowned for its hospice, palliative care, and community grief counseling programming is working with state and federal agencies to expand its services through Care Guide Partners to bring the national Program of All-Inclusive Care for the Elderly (PACE) to Hardin and surrounding counties. The unique PACE model was developed in 1971 to provide comprehensive care for people 55 or older who wish to maintain a healthy and independent lifestyle from the comfort of their current home.

The Medicare and Medicaid funded program provides a “one-stop shop” of complete managed healthcare and service delivery. An interdisciplinary team of specialists works with participants and families to develop a personalized plan of care to address each person’s unique medical, social, physical, and functional needs. All care is coordinated and monitored by the team to ensure participants have the support they need to age in place safely. Many services are offered at PACE centers, including primary medical care at the on-site clinic, adult day programming, occupational, physical, and recreational therapy, behavioral health, specialty services, hot meals, nutritional counseling, social activities, and more. The program provides roundtrip transportation from participants’ residences to all appointments managed by PACE to make accessing care as easy as possible for seniors and their families.
