Program of All-Inclusive Care for the Elderly (PACE)

Care Guide Partners PACE, a Hosparus Health affiliate, is working with state and federal agencies to bring the nationally recognized Program of All-Inclusive Care for the Elderly (PACE) model of care to the Elizabethtown region. Construction on the Care Guide Partners PACE Center, which serves as the hub for the program’s services, is on track for completion by early 2024.

The estimated start-up cost for the program is $11 million. Of this total, we need to raise $3 million through fundraising efforts and donor support. We are currently raising funds to establish an endowment to ensure the long-term financial stability of this project. We look forward to sharing more with you soon about how this innovative program allows our nonprofit organization to build on our 45-year mission and enhance the quality of life for those we serve.

Care Guide Partners PACE is currently undergoing the review process for new providers through the Centers for Medicare & Medicaid Services (CMS). Care Guide Partners does not yet offer PACE services and is not currently an approved PACE provider. This information is for educational purposes only.
