Hosparus Health offers a range of professional services focused on improving the quality of life for patients and their families throughout the continuum of serious illness care. Our team approach to holistic wellbeing ensures you and your loved one’s goals are our main priority. Services include:
Hospice Care
Hospice care is for people facing terminal illness with a life expectancy of six months or less. Hospice services are not curative but provide relief from pain and other symptoms associated with terminal illness. If you or a loved one has a life-limiting illness, our compassionate professionals are here to help.
Grief Support
For anyone suffering grief from loss or anticipated loss, Hosparus Health Grief Counseling Services are available to community members of all ages, regardless of prior engagement with Hosparus Health. Trained professionals offer grief support groups, individual and family counseling sessions, and community education. Counselors work with individuals to determine the best option based on need. If you or a loved one are ready to speak with a grief counselor, we’re here for you, call 502.456.5451 or 888.345.8197.
Program of All-Inclusive Care for the Elderly
Through Care Guide Partners, we offer the Program of All-Inclusive Care for the Elderly (PACE) services at our PACE Center in Elizabethtown. This model uses an interdisciplinary approach to meet the holistic care needs of seniors who wish to age in place safely from the comfort of their homes.
