Membership in the Autumn Leaf Society is a special distinction for individuals who notify Hosparus Health that they have made provisions in their estate plans to include support for the organization.
One simple way to join the Autumn Leaf Society is by including a gift to Hosparus Health in your will. Our partner, FreeWill, has a secure online tool you can use to make or update your will in just 20 minutes at no cost.

I would like to support Hosparus Health, but have a limited estate. Is there a minimum gift to he a member of the Autumn Leaf Society?
No. All gifts are welcome and will benefit patients and families now and in the future.
What language can I use for my bequest?
Here is a sample of a general bequest, unrestricted as to purpose, for use in the state of Kentucky:
“I give ( ____ dollars) or ( ____ percent of my estate) to Hosparus Inc, (dba Hosparus Health), a Kentucky charitable corporation, for its general purposes.”
