Care Guide Partners, a part of Hosparus Health, is pleased to bring the national Program of All-Inclusive Care for the Elderly (PACE) to the region. PACE aligns with our 45-year mission to enhance the quality of life for those we serve by delivering comprehensive care tailored to meet the holistic needs of today’s seniors.
We invite you to tour our innovative new PACE Center, featuring an onsite clinic, adult day program, therapies, personal care, nutritional counseling, and more.
